Turning 65 Medicare Checklist: Learn the Basics and Enroll

Published October 23, 2026

An older couple looks closely at a document being held by a Medicare advisor across a desk.

Approaching 65 is an important milestone for most Americans, and it can be an overwhelming time. You may already receive dozens of targeted Medicare advertisements months in advance. But what is the best choice?

For budget-conscious seniors looking for all-in-one healthcare convenience, Medicare Advantage (Part C) offers a good solution. These plans combine hospital and medical coverage with prescription drug protection — often for a $0 or low monthly premium — while adding valuable dental, vision, hearing, and other supplemental benefits.

Follow this step-by-step checklist to navigate your enrollment smoothly.

Medicare enrollment timeline

Your primary opportunity to sign up for Medicare is your Initial Enrollment Period (IEP). During this 7-month window, you can sign up for Original Medicare (Parts A and B) and enroll in a private Medicare plan. Don’t wait for your actual 65th birthday to start browsing and learning about Medicare plans!

Your 8-step Medicare Advantage enrollment checklist

Step 1: Calculate your Initial Enrollment Period (IEP)

  • Identify the exact dates of your 7-month IEP window (3 months before your birthday month, your birthday month, and 3 months after).
  • Marking these dates on your calendar ensures you don’t miss key deadlines.

Step 2: Decide whether to delay Part B (if still working)

  • If you or your spouse are still actively working at 65 and have coverage through a group health plan, check with your employer to see if your coverage qualifies as creditable.
  • If it is creditable, you may choose to delay Medicare Part B without incurring late-enrollment penalties later when you retire.

Step 3: Create your “My Social Security” account

  • Set up an online account at SSA.gov.
  • If you are already receiving Social Security retirement benefits, you will automatically be enrolled in Original Medicare (Parts A and B).
  • If you are not yet receiving benefits, you must initiate your Medicare enrollment through the Social Security website.

Step 4: Enroll in Original Medicare (Parts A & B)

  • Part A (Hospital Insurance): Pays for hospital care, inpatient stays, and skilled nursing care.
  • Part B (Medical Insurance): Pays for doctor visits, outpatient care, lab tests, and preventive care.

Step 5: Verify doctor networks and prescription formularies

  • Check whether your preferred physicians and medical providers are in the plan’s network.
  • Compile a list of your primary care doctors, specialists, preferred pharmacies, and current prescription medications to ensure complete plan compatibility.

Step 6: Select and enroll in your preferred Medicare plan

  • Compare the plans available in your zip code to find the best balance of low out-of-pocket costs, prescription coverage, and dental/vision benefits.
  • Enrolling during your IEP ensures your coverage begins without interruption.

Step 7: Set up premium payments and activate coverage

  • Pay your monthly premiums through automatic deductions from your Social Security benefits, online Medicare account, bank draft, or check.
  • Once enrolled, set up your online member portal to start utilizing your wellness benefits, dental coverage, and health allowances immediately.

What documents do I need for Medicare enrollment?

Gathering your essential documentation before starting the enrollment process saves time and prevents unnecessary delays:

  • Social Security Card
  • Birth Certificate to prove your age
  • Proof of U.S. Citizenship or Legal Residency such as a U.S. passport or green card
  • Military Discharge Papers if you served
  • W-2 Forms or Recent Tax Returns to verify recent work status or income tiers
  • Current Group Health Insurance Details including start and end dates
  • Personal Healthcare Information:
    • Complete list of current doctors, specialists, and medical facilities
    • List of current prescription medications, dosages, and preferred pharmacy
    • List of desired specific services (e.g., comprehensive dental coverage, vision allowances, hearing aid coverage)

Why choose Medicare Advantage at age 65?

While Original Medicare covers hospital and doctor care, it leaves out-of-pocket gaps — including copays, coinsurance, and deductibles — with no maximum out-of-pocket cap.

Medicare Advantage (Part C) fills these gaps while offering financial protection:

  • Out-of-Pocket Safeguards: Unlike Original Medicare, Medicare Advantage plans establish maximum out-of-pocket limits to protect your budget.
  • Low Premium Options: Designed for budget-conscious seniors, many Medicare Advantage plans offer low or $0 additional monthly plan premiums.
  • Additional Benefits May Be Available (Check Coverage Details):
    • Dental: Cleanings, X-rays, fillings, and dental care allowances
    • Vision: Annual eye exams and frame or lens allowances
    • Hearing: Routine hearing tests and hearing aid coverage
    • Wellness: Gym memberships, health rewards, and over-the-counter (OTC) allowances for everyday health products for some plans.

What SmartMatch Medicare Advantage customer data tells us

When we looked at our customer database, we found that a small percentage of our Medicare Advantage policyholders enrolled in an MA plan during their initial enrollment. However, more enrolled in an MA plan within the first year.

  • Only 7.9% of our MA customers enrolled in Medicare Advantage during their Initial Enrollment Period (within 3 months of turning 65).
  • Including the full first year after turning 65, that number rises to 13.7%
  • The majority (67.5%) of MA customers first enroll more than a year after turning 65 — meaning many people switch to Medicare Advantage later, often from Medicare Supplement or Original Medicare.

Likely, these customers enrolled in Medicare Part A and Part B during their IEP and then realized later they needed additional coverage that had lower monthly costs. By enrolling in an MA plan when you’re first eligible, you can lock in lower premiums and out-of-pocket maximums compared to Original Medicare coverage alone.

What happens if you miss your Medicare enrollment window?

Failing to enroll in Medicare on time without creditable employer coverage can trigger long-term financial penalties:

  • Part B Late Enrollment Penalty: A 10% penalty added to your standard monthly Part B premium for each 12-month period you were eligible but didn’t enroll. This penalty remains for the entire time you are enrolled in Medicare.
  • Part D Late Enrollment Penalty: A 1% monthly penalty based on the national base beneficiary premium for each month you went without creditable drug coverage. This penalty is added permanently to your monthly drug coverage fee.
